This video demonstrates a uniportal VATS middle lobe lobectomy in a 56-year-old female patient with a primary early-stage lung adenocarcinoma. After complete investigation via CT scan, PET scan, bronchoscopy, and pre-operative work-up, the patient was offered surgical treatment. During the procedure, the authors encountered an unusual anatomy of the pulmonary veins. In particular, the middle lobe drained to the superior and inferior pulmonary veins. The authors present this video to reinforce the importance of complete dissection of the pulmonary hilum before proceeding to the transection of the vascular branches.
