This video demonstrates a hybrid procedure for the closure of a large muscular ventricular septal defect in a 4-month-old boy with a body weight of 5.7 kg. After a median sternotomy, an 8F sheath is placed through the right ventricular free wall. An Amplatzer muscular VSD-occluder (8 mm) is implanted under echocardiographic visualization. This direct approach is very useful in small patients with apical defects and difficult vascular access. The duration of this procedure is short, and it can be accomplished at a low risk.
