This step-by-step video shows how to perform a mitral valve repair via inferior mini sternotomy. Bretschneider cardioplegia was used. The length of the neochords should be determined by considering the following factors: orientation to the neighboring chordae, orientation to the corresponding leaflet, the mitral valve ring, and experience of the surgeon. In principle, there are no contraindications to this approach. In cases of re-do operations, however, the approach should be guided by the preoperative CT images.
