Aortopulmonary window associated with interrupted aortic arch is a rare malformation. This video shows the one-stage repair of the defect in a 2.4 kg neonate. Cardiopulmonary bypass was established by using double arterial cannulation for uniform cooling and antegrade cerebral perfusion during hypothermic arrest.
The aortic arch was reconstructed by direct end-to-side anastomosis of the descending to ascending aorta and a pericardial patchplasty for closure of the aortopulmonary window. The pulmonary artery was closed in a direct fashion. The patient was extubated at postoperative day 2 and had an uneventful postoperative course.
