Surgical management of malignant pleural mesothelioma remains a challenging task. This video tutorial demonstrates the surgical anatomy along the extrapleural plane in the right chest, the pathological features of malignant pleural mesothelioma and the techniques of cytoreductive surgery, ie., extrapleural pneumonectomy for a patient with right-sided malignant pleural mesothelioma. Extrapleural pneumonectomy consists of apical extrapleural dissection, mediastinal extrapleural dissection, diaphragmatic extrapleural dissection, hilar control, pericardial resection/reconstruction and diaphragm resection/reconstruction.
