An asymptomatic 51-year-old man with bilateral pulmonary artery dilatation, incidentally noted on chest radiography 20 years previously, demonstrated progressive dilatation up to 5.8 cm on serial imaging studies despite normal pulmonary artery pressures. The aneurysm involved the left and right branch pulmonary arteries and spared the main pulmonary artery and the pulmonary valve. The patient underwent replacement of his left and right PA using a 22 mm Dacron graft. The midportion of the Dacron graft was anastomosed to the stump of the main PA. The patient’s postoperative stay was uneventful and he was discharged on postoperative day four.

Interesting video demonstration. I wonder if one would have started with LPA anastomosis before the RPA side? I believe, Ascending aortic transection & reanstomosis is an option to enhance the exposure if need be.
