This video demonstrates a valve-sparing root replacement (David procedure) with removal of the papillary fibroelastoma and reconstruction of the valve using 180° commissure orientation and cusp plications. A 34 mm straight Dacron graft was used. The base of the prosthesis and the coronary ostia were marked with radiopaque markers.

Bicuspid valves with plication may have reduced long term results especially in patients with annulus over 30 mm.
