VATS is a safe approach for the treatment of a Pancoast tumor. When combined with other accesses, a VATS approach allows for radical resection, with the advantages of improved wound healing, better postoperative pain control, and decreased respiratory complications. This video presents the case of a 57-year-old patient with an adenocarcinoma of the superior sulcus in the right upper lobe, who was scheduled for surgery four weeks after induction therapy (CT and RT). VATS combined with a posterior thoracotomy approach provides the ability to assess the optimal incision and extent of rib resection. This reduces surgical trauma and provides very good results in terms of postoperative morbidity and disease-free survival.
